Add a condition to the problem list

Record a long-term condition on a patient's problem list, with the date it started.

The problem list keeps a patient's long-term conditions at the top of the chart, so every clinician sees them without reading back through old notes.

Steps

  1. Open the patient's chart.
  2. Choose Record a condition.
  3. Type the condition in Condition.
  4. Under Began on, if known, fill in the Day, Month and Year it started.

  1. Choose Record the condition.

What to expect

The condition is listed under Problem list with the date it started and who recorded it.

Beside each condition is Resolve or correct, for when the condition ends or was recorded wrong.

  • Record a patient's allergiesRecord what a patient is allergic to, or that they know of no allergy, so it shows at the head of their chart.
  • Open a patient's chartRead a patient's chart, with their allergies, problem list, latest signed note and every past visit.
  • Write and sign a noteWrite a consultation note from a template during a visit, check it, and sign it.
