Blank worksheet: Hospital service price change register
First worksheet at a glance. The complete blank worksheets are below.

For practice managers and staff responsible for the service price list.

The poster has been changed, but the billing desk still uses the old consultation price. Before a revised price takes effect, record the approval and every place staff must update. Keep the effective date clear so a later bill can be checked against the price that applied.

Write amounts in naira and identify the exact service. This is a change-control worksheet, not a recommended tariff or a price-setting formula. Agree the hospital's charging policy before updating records, including how quotations already given will be handled.

How to use it

Define exactly what changes

Give the service a stable code and a clear description. Record old and proposed prices, the reason and the intended start date and time. If the scope changes as well as the price, describe that difference explicitly. A consultation bundled with another service cannot be compared with a consultation alone simply because both labels look similar.

Approve the effective rule

The authorised manager should decide how the change affects existing appointments, quotations and bills. Do not silently recalculate historical invoices from today's price list. If an existing bill needs correction, use the hospital's documented correction process and preserve its reference. Record the decision before the change is applied, including any exception the billing team must recognise.

Check each place staff use the price

Update the controlled price list, approved patient information and billing configuration that actually apply. Use a fictional transaction to check the new price and confirm an earlier invoice still shows its original amount. Have the next shift acknowledge the effective rule. Keep unresolved locations on the register until someone verifies them.

The FAO-hosted manual discusses general financial records in Liberia. It does not set healthcare prices in Nigeria. The approval, effective-date and communication checks here are RizCare's proposed operating routine.

What each field means

Service code and scope
Keep a stable identifier alongside the name. State whether the price is for one visit, one item or a defined bundle so staff do not apply it to a different service.
Old and approved price
Write both amounts and the currency. An approved price differs from a proposed amount awaiting a decision; do not publish the proposal as the current price.
Effective rule
Include the date, time and treatment of previously issued quotations. An effective date on its own does not answer every question about an already agreed bill.
Applied and checked
List the locations updated and the person who observed the result. A saved setting is only one location if a printed list is still used at reception.

Completed fictional example

The prices and policy in this example are invented. They are not a Nigerian market estimate or a recommended consultation fee.
