RizCare's suggested worksheet. Adapt to your hospital. https://riz.care/library/templates/clinic-billing-query-log
For billing staff and the practice manager.
A patient points to a second line on the invoice and asks why it is there. Write down that specific question beside the bill reference. The next colleague needs to see what has been checked and who can decide the explanation or correction.
This suggested routine is for Nigerian hospitals handling cash and bank-transfer payments. It tracks an investigation; opening a query does not cancel a bill or approve a refund. Continue any needed clinical care through the hospital's usual escalation process, separately from the billing discussion.
Print this page or save it as a PDF from your browser. The print layout contains only the blank worksheet and its completion note. Fill it in within your hospital.

How to use it
Write down the specific question
Record which bill line is disputed and what the patient believes happened. Distinguish an unexplained charge from a payment said to be missing. Read the question back without promising an adjustment before the responsible person checks the evidence. Use an internal reference on the log, keeping detailed patient information in the authorised record.
Check the underlying records
Compare the dated service entry, price used, invoice and relevant receipt. If the question concerns a transfer, refer to the transfer investigation log and the authorised bank check. A payer's screenshot alone does not establish a receiving credit. Preserve the original invoice while the query remains open; an unexplained difference is not a reason to overwrite it.
Record the decision and close the conversation
Name the person authorised to explain or correct the bill, the evidence considered and the decision date. If a correction is approved, record its separate reference and check the resulting balance. Record how the patient was informed and any question left unresolved. Close the query only when these actions are complete.
The cited manual covers general bookkeeping records in a Liberia forestry setting. It provides background for keeping supporting references, not a Nigerian hospital complaint procedure. This log and its response steps are our own operational recommendations.

What each field means
- Query reference
- Give the conversation its own reference. Several queries can concern one invoice, so using the invoice number alone can hide which question has been resolved.
- Evidence checked
- Name the record and date reviewed, rather than writing only checked. Do not copy clinical notes or a full bank statement into this log.
- Decision and authority
- State explanation given, correction authorised or still under review. Record an actual authorised decision; a staff member receiving the query may not have that authority.
- Patient follow-up
- Record the agreed contact route in the protected patient record and the communication outcome here. A sent message is not evidence that the patient understood the explanation.
Completed fictional example
Fictional references illustrate two different outcomes. A correction reference is not a cash refund, and the open transfer query does not count as a received payment.
| Query / invoice | Question | Evidence / decision | Owner / follow-up |
|---|---|---|---|
| Q-01 / INV-021 | Two consultation lines queried | Service record checked; duplicate line referred for correction C-03 | Manager A; explanation given 9 September |
| Q-02 / INV-029 | Transfer said to be missing | Transfer enquiry T-07 remains unmatched; no second receipt entered | Billing B; next check 10 September |
