When a patient questions the quantity on a hospital bill

Investigate a disputed quantity by following the invoice back to the authorised service record, then document the explanation or approved correction.

The invoice says two consultations. The patient says they attended once. Start with that specific disagreement and the records behind it. Telling the patient that the computer calculated it will not explain why the quantity is two.

Give the question an owner through the billing query log. The staff member receiving it can record the concern without promising a discount or making a clinical judgement about which care was necessary.

Find the unit being charged

Check the service name, quantity, unit price and invoice date. Ask what one unit represents under the hospital's approved price list. A package, an attendance and an individual service may have different charging rules, and those rules should be established before staff explain the amount.

Retrieve the authorised supporting service entry. If the invoice and service record disagree, describe the difference plainly. Avoid copying clinical notes into a general billing enquiry. The responsible clinical team should clarify clinical documentation through its own record process.

Work through the actual discrepancy

In a fictional example, two identical 5,000-naira lines were added against one documented billable service. The reviewer confirms that one line was duplicated. The proposed bill reduction is 5,000 naira, with the original invoice and investigation preserved.

In another case, two separately documented services explain the quantity. The billing lead can show the authorised explanation in appropriate terms. A query ending with an explanation is a valid outcome; staff should not feel obliged to alter a correct bill merely to close a complaint.

Record the decision the hospital made

Use the adjustment authorisation record when a specific correction is proposed. Record the approving person, actual correction reference and resulting balance. If the patient already paid, any question about returning money goes through the separate refund procedure.

The FAO-hosted bookkeeping manual supplies general background on supporting business records. The investigation routine here is a suggested hospital process, not a rule prescribing which services a clinician should bill.

Tell the patient what was checked and what happens next through the authorised contact route. Leave the query open if the reviewer still needs a document or decision, with a named colleague responsible for obtaining it.
