Payment reversal and refund: check the record and the money
Distinguish correcting an incorrect payment entry from returning money, and keep the separate evidence visible when a hospital reviews a billing problem.

A receptionist entered a 10,000-naira payment twice. The hospital received 10,000, but the record shows 20,000. Correcting the duplicate entry should leave a clear history of what went wrong. There is no extra 10,000 in the hospital's bank account to return.
Now consider a different fictional case: the hospital actually received an excess payment and an authorised person decides to return it. That requires the hospital's refund procedure and evidence of the money returned. The word reversal can conceal this difference if staff use it for both situations.
Establish what happened outside the software
Find the original receipts and the authorised bank or cash evidence. Identify how many payments were received and how many entries were recorded. Keep a disputed or unconfirmed transaction under review until the responsible person establishes the facts.
Ask separately whether the bill itself is wrong. Removing a duplicate charge, correcting a payment record and authorising money to leave the hospital require different evidence and may require different permissions.
Correct a record with its reason
In RizCare, an authorised administrator can reverse an incorrect payment record with a reason while the original remains in the history. That action does not refund money. Check the resulting invoice balance and retain the correction reference for the next colleague who explains it.
Do not delete supporting evidence or create another payment merely to make the displayed total look right. If the proposed correction does not match the task the system supports, raise it with the responsible administrator instead of improvising a sequence of unrelated entries.
Follow a refund through its own process
The hospital decides who may authorise a refund and what checks apply. Record the decision separately from the completed return of money. A refund requested, approved and paid can be three different stages of one enquiry.
The bookkeeping manual hosted by FAO provides general record-keeping background. It does not define Nigerian hospital refund rights or a bank's dispute process.
Use the billing query log to connect the investigation, correction reference and any separate authorised refund evidence. Before closing the query, have the responsible colleague explain both the remaining bill balance and what money actually changed hands.
