For practice managers and staff reviewing reception and consultation flow.
Ask two observers when a patient started waiting and they may give different answers. One began at arrival; the other began when registration finished. Choose the interval before using this sheet so the times describe the same stage of a visit.
This is a small operational observation exercise for Nigerian hospitals, not a national waiting-time benchmark or evidence of a software benefit. Use anonymous observation references, explain the exercise to the team and follow the hospital's approved arrangements for observing routine work.

How to use it
Define the events and observation window
Write the start event, end event, date and time window. Use one clock or check that observers' clocks agree. State which visits are eligible and how you will record an incomplete observation. Select consecutive eligible visits within the window where practical, rather than only those that moved quickly or had an uncomplicated journey.
Record observed times and context
Capture the actual event times without interrupting care. Separate waiting for a service from time spent receiving it. Record relevant operational context, such as a clinician arriving late or a registration system interruption, without adding diagnoses. Urgent care follows clinical priority; this exercise must not change that priority or encourage staff to delay a needed escalation.
Calculate and interpret the interval
Subtract the defined start time from the end time. Use full dates when an interval crosses midnight. Keep missing endpoints visible and exclude them from a completed-interval summary with an explicit count. Before comparing two windows, check whether the events, eligible visits and staffing conditions were comparable. A shorter interval in a small sample cannot by itself establish a cause.
AHRQ's US toolkit covers assessing workflow around health IT. The event definitions and worksheet here are our suggested local observation method, not a validated Nigerian performance target.
What each field means
- Observation reference
- Use a temporary anonymous code that does not reveal patient identity. This worksheet should not become a second patient register or a list of clinical reasons for attendance.
- Start and end event
- Name visible events consistently. Registration completed, folder delivered and consultation started are different moments; choose the pair that answers the question you are investigating.
- Elapsed minutes
- Calculate only when both endpoints were observed. Do not enter zero for a missing end time, because that would make the average wait appear shorter.
- Context and completeness
- Note interruptions and incomplete observations. Keep the original times so someone can recheck a calculation and distinguish a missing observation from a genuinely short interval.
Completed fictional example
The fictional completed intervals are 18 and 25 minutes, averaging 21.5 minutes. One observation is incomplete. These values are not a target, national estimate or measured hospital result.
| Observation | Start | End | Minutes / context |

| OBS-01 | 9 September 09:05 | 9 September 09:23 | 18; routine observation |
| OBS-02 | 9 September 09:10 | 9 September 09:35 | 25; consultation started later |
| OBS-03 | 9 September 09:18 | Not observed | Incomplete; excluded from completed intervals |
