RizCare's suggested worksheet. Adapt to your hospital. https://riz.care/library/templates/clinic-staff-training-signoff
For practice managers, trainers and staff learning a hospital workflow.
Let the colleague perform the task while the trainer watches. Following a demonstration together cannot show whether someone can find the right patient or explain a part payment alone. Record the help needed and the next supervised exercise beside the task observed.
Choose tasks that belong to the staff member's approved role in your Nigerian hospital. Use fictional practice records in an approved training environment. A training sign-off does not grant access, certify a professional skill or authorise work outside the person's role.
Print this page or save it as a PDF from your browser. The print layout contains only the blank worksheet and its completion note. Fill it in within your hospital.

How to use it
Choose observable tasks
Describe what the learner must do from start to finish: identify the right fictional record, complete the intended entry and explain what remains unfinished. Choose a few tasks from the person's daily work instead of listing every menu. Record the environment and role used, since success with an administrator account does not establish what a receptionist can do.
Observe an attempt without taking over
Ask the learner to carry out the task and say what they would check before saving. Record completed independently, completed with prompting or awaiting demonstration. If the trainer takes the mouse or supplies the answer, do not label the attempt independent. Include a common exception, such as a similar patient name or a payment covering only part of a bill.
Agree the next practice and review
Give each gap a specific practice task and a reviewer. Repeat the relevant observation after practice and retain the earlier result so improvement is visible. Let the manager decide what supervision is needed meanwhile. Avoid using speed alone as the success criterion when correct identity and accurate records are part of the task.
The Nigerian primary-care case study identifies training and local implementation context as important considerations. It does not validate this sign-off sheet or prove private-hospital outcomes; the observation routine here is our recommendation.
What each field means
- Task and role
- Name a complete task and the actual account role used. A task with broader permissions may need a different trainer or should be excluded from this person's programme.
- Observed result
- Describe the action seen, including any mistaken selection caught before saving. Avoid recording only trained, confident or understands when no attempt was observed.
- Help needed
- State the prompt or intervention. A task completed with help can still guide the next session, but it should not be presented as independent performance.
- Next practice and review
- Set the next attempt, the responsible reviewer and a locally agreed date. Leave the task open if access or a suitable training environment prevented observation.
Completed fictional example
The tasks, amounts and observations are fictional. A correct balance in this example is arithmetic, not evidence that a staff member has completed training.
| Task / role | Attempt observed | Help needed | Next review |
|---|---|---|---|
| Find fictional patient / reception | Selected correct record after checking identifiers | One prompt to check the second identifier | Repeat with another example; Trainer A |
| Record part payment / billing | Correct 5,000 receipt against 8,000 bill; 3,000 remains | None | Observed independent by Trainer B |
| Explain an unresolved transfer / billing | Awaiting demonstration | Further practice needed | Use unmatched-transfer scenario; Trainer B |
