For staff issuing receipt copies.
A patient has lost a receipt and asks for another. Before printing anything, find the original payment entry. A copy should lead back to the money already received; entering a new payment to produce a fresh receipt would increase the recorded collections.
Keep this register beside your authorised billing process. It records the decision to issue a copy and its destination. The patient record or invoice remains the place to find the underlying charge.
Print this page or save it as a PDF from your browser. The print layout contains only the blank worksheet and its completion note. Fill it in within your hospital.

How to use it
Find the original
Ask for enough information to locate the invoice through your normal identity checks. Compare the requested amount and date with the original receipt. If the original cannot be found, open a billing query and tell the requester who is checking it. A remembered amount is insufficient evidence for recreating a receipt.
Issue a clearly identified copy
Have the authorised staff member approve the copy. Preserve the original receipt reference and amount, and mark the document as a copy using the hospital's agreed wording. Record whether it replaces a damaged printout or is an additional copy for an authorised purpose. Check the recipient before handing it over or sending it.
Account for the request
Enter the copy reference and delivery outcome here. No cash or transfer is added to the day's collections for this action. If someone discovers an incorrect original payment, send that for a separate correction with its own reason. A reprint must not quietly change the payment history.
What each field means
- Original receipt
- Use the full reference that retrieves the existing entry. A patient name alone can point to several receipts on different dates.
- Reason and authority
- Write the practical reason for the request and the approving staff member. Do not assume that anybody holding a receipt number may receive a copy.
- Copy destination
- Record the authorised recipient or protected delivery reference. Keep phone numbers and other contact details in the patient record rather than repeating them on this sheet.
Completed fictional example
Fictional receipt RC-040 remains the original payment. COPY-01 adds no money to collections.
| Request date | Original receipt | Reason / approval | Copy / delivery |
|---|---|---|---|
| 9 September | RC-040 | Damaged printout; Manager A | COPY-01; handed to authorised requester |
