For staff making authorised administrative callbacks.
A receptionist calls about a billing explanation and reaches voicemail. At handover, called can mean several things: the phone rang, someone answered, or the intended person received the message. Record the outcome that actually happened.
Use this sheet for authorised administrative contact. Clinical follow-up and communication of results need the hospital's clinical procedures and responsible clinician.
Print this page or save it as a PDF from your browser. The print layout contains only the blank worksheet and its completion note. Fill it in within your hospital.

Keep completed copies with the staff authorised to use them.
How to use it
Check the contact instruction
Find the agreed purpose and permitted contact route in the protected patient record. Confirm who may receive the information, including whether a household number is shared. Decide what a staff member can say if somebody else answers. Do not place sensitive details into an unsolicited message simply because a number appears on the registration form.
Record each attempt plainly
Enter the time, the authorised channel and the observed outcome. Use descriptions such as no answer, message left as permitted, or intended recipient reached. Record any administrative question that needs a separate follow-up by reference. Avoid copying the conversation or contact details onto a sheet that may move between desks.
Agree the next action
If the intended person was reached, record whether the administrative purpose was completed or another conversation was agreed. If contact failed, follow the hospital's policy on further attempts and escalation. This form sets no universal number of calls or deadline. At shift change, have the incoming owner acknowledge any action that remains.
What each field means
- Contact task
- Use a reference that retrieves the purpose and authorised contact details. A reference alone must not give a staff member permission they do not otherwise hold.
- Outcome
- Describe delivery accurately. A sent message and a completed conversation are different observations, and neither proves a clinical instruction was understood.
- Next owner and date
- Record the person accepting the remaining administrative task and the agreed review point. Leave clinical urgency decisions with the clinical team and its established escalation process.
Completed fictional example
Fictional attempts concern an administrative billing conversation. No patient phone number or result detail belongs on this example.

| CALL-08 | 9 September 10:10 / agreed phone | No answer | Try at agreed time / Reception B |
