RizCare's suggested worksheet. Adapt to your hospital. https://riz.care/library/templates/clinic-implementation-responsibility-matrix
For the hospital lead coordinating an agreed software implementation.
The supplier is waiting for the hospital's service list. The hospital is waiting for setup to finish. Both can be busy while the same dependency remains unresolved. Write down who supplies what and who accepts the result.
Build this sheet from the agreed scope. It should clarify responsibilities already authorised, not add features or services that nobody has agreed to provide.
Print this page or save it as a PDF from your browser. The print layout contains only the blank worksheet and its completion note. Fill it in within your hospital.

Keep completed copies with the staff authorised to use them.
How to use it
Break the work into deliverables
Choose an outcome that another person can inspect: an approved price list supplied, named staff accounts arranged, or a training session scheduled. Avoid a single row called complete implementation. Record the hospital owner for each input and the supplier contact where relevant. Keep passwords and patient data out of a shared planning sheet.
Expose the dependencies
Write what must happen before the task can begin, including a required decision or a document the hospital must prepare. Ask each assigned person to acknowledge the action and date. If one dependency moves, revisit the affected dates with those people instead of leaving the plan to imply that every other task remains on time.
Accept completed work against the agreement
Specify the evidence that lets the hospital accept each deliverable. A session booked is different from staff attending, and staff attendance is different from an observed task assessment. Record open questions with an owner. The authorised hospital lead decides whether the relevant readiness conditions are met before staff change their working routine.
What each field means
- Deliverable
- Describe something observable that fits the signed scope. A requested custom report stays a request until both parties agree who will provide it.
- Responsible and accepting people
- Name the person doing the work and the person checking completion. Supplier delivery does not automatically mean the hospital has accepted the result.
- Dependency and evidence
- Link the required input and completion evidence by reference. Record a blocked task honestly and identify the decision that will unblock it.
Completed fictional example
The invented responsibilities do not describe a RizCare service commitment. Use the scope agreed for your own implementation.
| Deliverable | Responsible person | Dependency | Acceptance condition | Status / next action |
|---|---|---|---|---|
| Approved service list | Hospital billing lead | Owner price decision | Dated list checked by hospital lead | Awaiting decision |
| Training session | Agreed supplier contact | Staff availability | Session held; questions logged | Date to agree |
