RizCare's suggested worksheet. Adapt to your hospital. https://riz.care/library/templates/patient-demographic-correction-request
For reception staff and authorised record administrators.
A patient points out a misspelt name on their record. Confirm that the record belongs to the right person, then document the requested correction and the evidence checked. Receiving the request and approving the change are separate steps, even when the spelling error looks obvious.
This is an administrative worksheet for Nigerian hospitals. It is not a statutory request form or legal advice about response deadlines. Requests involving clinical notes, disclosure, deletion or a disputed identity need the hospital's appropriate review process rather than a routine demographic edit.

How to use it
Confirm the person and the record
Check identity using the hospital's process before opening a correction. Record whether the request comes from the patient or someone acting for them, and how that person's authority was checked. A person who can pay a bill or answer a shared telephone is not automatically authorised to change the patient's details. Escalate uncertainty before applying an edit.
Capture the requested change precisely
Name the field and record the existing and proposed values in a protected working copy. Ask whether the existing value was wrong when entered or whether the detail has changed since then. Record the source of the request and any supporting evidence needed by the hospital. Do not collect extra identity documents simply because there is space on a worksheet.
Apply and check the authorised outcome
An authorised reviewer should record approval, refusal with an explanation, or a request for more evidence. Use the software's supported correction route and preserve the change history it provides. Check the displayed result and any related administrative list that needs attention. Do not rewrite a signed clinical note to make it match a newer demographic value.
WHO patient-identification guidance supports confirming the person before record actions. It does not prescribe this correction form or establish Nigerian legal rights or deadlines; the review workflow here is RizCare's recommendation.
What each field means
- Record and requester
- Record the internal patient reference and requester relationship. Keep a note of the authority check in the approved location, without including unnecessary personal details in a handover list.
- Existing and requested values
- Name one field per row. Distinguish a corrected original value from a newly current value so a later reviewer can understand the reason for the change.
- Evidence and decision
- Name the evidence actually considered, the reviewer and decision time. Request received and correction approved are separate events.
- Applied and verified
- Record who made the change, its reference and who checked the outcome. Mark follow-up work explicitly if an old printed list remains in circulation.
Completed fictional example
The example uses a fictional reference and omits contact values. In a hospital, the controlled request must identify the exact values; a general handover log need not repeat them.
| Field | Entry |
|---|---|
| Record / request | P-218 / D-06 |
| Field / reason | Contact preference; detail has changed |
| Requested change | Use the new contact route held in the protected record |
| Review | Requester and authority checked by Administrator A |
| Outcome | Approved; change CH-11 applied and checked |
| Follow-up | Reception list updated; patient informed |
