RizCare's suggested worksheet. Adapt to your hospital. https://riz.care/library/templates/clinic-paper-record-movement-register
For records staff and teams still moving paper folders within the hospital.
The consultation room has finished with a folder, but the records clerk cannot find it. Record each transfer when it happens so staff can identify the current holder and expected return. The movement register needs a folder reference and custody trail, without a copy of the clinical history.
The register suits a Nigerian hospital keeping paper alongside digital records. It tracks internal custody only. Sending a record outside the hospital, copying it for another organisation or responding to a disclosure request needs the hospital's separate authorised process.
Print this page or save it as a PDF from your browser. The print layout contains only the blank worksheet and its completion note. Fill it in within your hospital.

How to use it
Record the handover at the point of movement
Use the folder reference, time out, receiving staff member and destination. Confirm that the receiving person actually has the folder rather than marking it issued when someone merely asks for it. Keep a placeholder in the filing location if that is part of the hospital's existing process, so another staff member does not assume the record has disappeared.
Keep subsequent moves visible
When the folder passes to another authorised work point, add the next movement with a time and recipient. Do not erase the first recipient or ask staff to reconstruct several moves at the end of the day. Use the minimum identifying information needed to find the record, and keep the movement register out of public view at reception.
Check returns and unresolved folders
Record the actual return time and who confirmed the filing location. A folder found on a desk does not establish a confirmed return to storage. Review overdue movements at handover, assign an owner and follow the hospital's missing-record process when a folder cannot be located. Do not mark returned simply to complete the day's sheet.
AHRQ's US toolkit addresses the relationship between health IT and administrative workflow. This internal folder-custody routine is our own recommendation for hospitals working across paper and digital processes.
What each field means
- Folder reference
- Use the hospital's stable folder identifier and check it on the folder being moved. Avoid writing diagnostic information into a movement register.
- Recipient and destination
- Name the staff member receiving custody and the work point. A room label alone cannot tell the next shift who can answer a question about the folder.
- Time out and time returned
- Record observed handover times, including the date for movements across shifts. An expected return is a plan and belongs separately from an actual return.
- Unresolved action
- Record the last confirmed holder, next search step and responsible person. Keep an open movement open until the folder is located and its custody confirmed.
Completed fictional example
The folder references and staff labels are fictional. The open row shows known custody and an assigned follow-up, not proof that a folder is lost.
