RizCare's suggested worksheet. Adapt to your hospital. https://riz.care/library/templates/clinic-external-lab-directory-review
For the colleague maintaining the hospital laboratory contact directory.
A referral goes to an email address the laboratory stopped using. Staff can spend the next shift chasing a report that never entered the intended channel. Keep the hospital's authorised directory current enough for the person making the next request.
The directory records administrative contacts. The hospital's clinical and procurement leads remain responsible for deciding which laboratories may receive its work.
Print this page or save it as a PDF from your browser. The print layout contains only the blank worksheet and its completion note. Fill it in within your hospital.

Keep completed copies with the staff authorised to use them.
How to use it
Check an existing approved entry
Start with the hospital's authorised laboratory list. Confirm the department or role that accepts requests and the channel staff should use. Ask how to route a query about an existing request and where administrative availability is recorded. Avoid copying personal staff numbers from an old conversation into the official directory without confirmation.
Record the evidence for a change
Write the date, the laboratory contact or official source used, and the person who checked it. Keep the old entry traceable when a contact changes. If details conflict, flag the entry for the responsible lead instead of selecting whichever number answers first. A current phone number does not establish laboratory accreditation or clinical suitability.
Distribute the corrected directory
Tell authorised users which entry changed and replace the controlled copies through the hospital's document process. Check printed desk copies as well as the central file. The WHO laboratory handbook provides background on referral records; it does not endorse a laboratory or define this review schedule. Follow the receiving laboratory's current instructions for any specimen requirements through the clinical team.
What each field means
- Directory reference
- Use the existing hospital reference for the laboratory. A new contact person should not create a second laboratory entry with an indistinguishable name.
- Verified channel
- Record the administrative purpose and its confirmed route. Store detailed contact information in the controlled directory and use its reference on the review sheet.
- Reviewer and follow-up
- Record who checked the information, when it was checked and what remains uncertain. Choose the next review point under local policy and revisit it after a failed contact.
Completed fictional example
These are invented directory references. They identify no real laboratory or claimed approval.
