Registering a follow-up visit without creating a second patient

Find the existing patient, check which attendance the team needs and open the returning visit through the hospital routine without duplicating identity.

A patient comes back as instructed after a previous consultation. Reception needs to find the existing person and establish today's attendance. Creating a second patient because today's visit is new makes the earlier record harder for the team to reach.

Start with the patient search and the hospital's identity checks. A follow-up description tells staff why the person has returned; it does not establish which record on screen belongs to them.

Find the existing person

Ask for the usual identifiers, including the folder card where available. Search carefully if spelling or contact details differ. The WHO identification guidance underpins the identity check; the desk sequence here is our operational recommendation.

If the record is unavailable on the current device, use the returning-record search routine. Give the search an owner while the hospital follows its agreed continuity procedure. Do not fill a new patient record with remembered details merely to get past the search screen.

Check the visit being opened

Confirm the attendance date and the appropriate queue or team through the hospital's working procedure. Check whether another receptionist has already registered this attendance before opening it again. An entry missing on one device may require a status check, especially during an outage.

Keep the previous consultation accessible as history. The responsible clinician decides how to document the return and any clinical relationship to earlier work. Reception should not copy an old note forward or decide that the previous treatment remains current.

Handle administrative changes separately

A new phone number needs an authorised demographic correction. A question about whether the visit is included in a package needs the agreed billing terms. Neither question should be answered by changing the person's identity.

For a fictional patient with folder F-028, today's return can therefore involve an existing patient, a new attendance and a contact update. Each has its own purpose. A payment against an older bill remains connected to that bill even if it is received during today's visit.

Before handing the patient to the next team, confirm that the intended attendance is visible through the authorised workflow and that any unresolved administrative question has a named owner. Leave clinical decisions for the clinician receiving the visit.
